Application Process

COMMUNITY CHRISTIAN SCHOOL
Enrollment Procedure Check List


1. Complete enrollment forms.
2. Return the forms to the office.

You will need to bring:
A. Completed enrollment forms:
1) CCS Application for Admission
2) Medication Form/ Grandparent Information
3) Parent Agreement
4) Partnership Agreement- (Grade 7 & up)
B. Other school records:
1) Most recent report card
2) Test Scores
3) Immunization records
C. Copy of:
1) Birth Certificate
2) Social Security Card
D. Completed Financial Aid forms if applicable and a copy of your previous year's Income Tax Form.
3. Schedule an appointment for placement evaluation.
Testing cost is $30.00.

4. Schedule an interview with the administrator. You will need to bring the required Registration Fee in the amount of $150.00 ($125.00 for each subsequent child).

5. When your child is accepted you will need to discuss a tuition payment plan. You may pay full tuition or set up a monthly payment schedule.
